All spanish residents are covered by social health service insurance. This applies as well to non spaniards that do reside legally in Spain. 

There are two manners in which you become a resident in Spain. The first if you are an EC (european Community) citizen, you do qualify for this immediately by entering any district police station and fill in a form stating that you are currently living in Spain. This will allow you to take employment as well.

For non EC citizens the process is more complicated, since one should apply for residency card, this process takes couple of months, and starts with the spanish embassy in your country.Some nationals have special treaties with Spain,which makes this process easier. 

Any way, once resident in Spain, you are given a social security number which will allow you to benefit from services given to spanish residents, this includes Health Services (free), and even unemployment benefits.
